*Important information please read these terms before signing Life Choices Foundation is committed to Youth Wellbeing and Health Programs and thanks to our sponsorship we are able to keep the fees low. To secure our attendance, however, we do require schools to adhere to the Terms & Conditions of the Agreement. Terms & Conditions:
Payment: Invoices are to be paid no later than 14 days prior to the booking date.
Cancellation Policy: Once your booking form has been received and the booking confirmed by Life Choices Foundation, a 21 day cancellation policy will apply. If the School cancels within this 21 day period, the full booking amount will be payable. However, if Life Choices Foundation is able to fill the vacated spot with an alternative booking, the cancellation fee may be waived at our discretion.
By returning this signed agreement, you also agree to have staff and students complete the feedback survey forms provided and return these within 48 hours unless otherwise agreed by us. Thank you for your booking. We are looking forward to bringing the Life Choices Program to your school!
